The first step in scheduling an evaluation is for the the child's parents to fill out an intake form and for a teacher who knows the child well to fill out a short questionnaire. All the necessary forms can be be downloaded here.

As a rule we are unable to schedule evaluations without both the parent and teacher forms  but please contact us for extenuating circumstances.
